Overview

CarePlus New Jersey is a northern New Jersey provider of recovery-focused mental health, substance abuse, and family services that address the unique needs of individuals with a holistic approach to recovery and overall wellness.

 

Operating out of 23 sites – including three outpatient centers located in Paramus, Fair Lawn, and Montclair, ten residential facilities, offices at three local hospitals, and seven community offices – our services are convenient to clients located throughout Northern New Jersey. We also provide educational programs on mental health-related topics to area schools and community groups.

Responsibilities

 

Provides direct case management services to an adult mental health population in their home environment with the goal of promoting community tenure and individualized wellness and recovery. Works directly with consumers to achieve stability in their lives and decrease reliance on emergency and hospital based services. Assists participants to use the resources within their community to promote independence in personal goals.

  • Assess comprehensive service needs (clinical, social, recreational and vocational) and provide the appropriate level of support and case management.
  • Provide customized services based on the participant's stated and felt needs.
  • Provide on-going reassessment every 90 days or as clinically indicated.
  • To increase participant's abilities to maintain stabilization within the community.
  • To support recovery by working to enhance the participant's physical health, spirituality, emotional well-being and social connections.
  • Ensure access to a system of support services and linkages that can assist in maintaining independence and desired lifestyle.

 

Service provision is via extensive outreach and comprehensive case management, advocacy, psycho-education, supportive counseling and the establishment of a recovery plan that will engage the consumer into linkages to requisite services.



Qualifications

  • BSW or BA in Mental Health or related field + 1 year post BA experience in mental health or related field
  • Extensive knowledge of community services
  • Must be able to work a flexible work schedule
  • Possess a valid driver's license and must be  insurable under Agency's automobile insurance policy
