CarePlus New Jersey is a northern New Jersey provider of recovery-focused mental health, substance abuse, and family services that address the unique needs of individuals with a holistic approach to recovery and overall wellness.

Provide care management services for Bergen County residents that are homeless that promotes permanent housing linkage, independence and stability in the community.  Provide a range of services varying from on-site intake and assessments to in-community neighborhood-based care management services following linkage with permanent housing.  Required to deliver services consistent with the Housing First, and Wellness and Recovery philosophies.    Work collaboratively with the Shelter/Drop-In Center operator as well as all other health and human services available on-site and in the community. Required to work a flexible schedule that includes evening, weekend and on-call responsibilities.


Carries caseload of at least 11 clients, as directed.

Assesses clients at intake, during ongoing service delivery and at discharge.

Collaborates with service providers to establish comprehensive assessments and care plans.

Coordinate continuity of care within the agency, community, jails, hospitals, etc.

Formulates comprehensive care plans containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs quarterly reviews of progress and evaluates effectiveness of goals/objectives.

Engages clients in services and accesses/ addresses their needs; provides or links to health care services, dental care, benefits, mental health services, substance abuse services, family consultation, crisis intervention and vocational/educational/employment services; etc.

Maintains case files on every assigned client.

Participates in Case Collaboration meetings with supervisor, collaborating agencies, as directed.

Link clients to community service providers that meet the client's service goals as well as achieve service outcome targets.

Act as liaison and maintain open communication with existing case management entities linked with the client.

Communicates client progress, needs and concerns through case review and monthly reports.

Completes ongoing evaluations/ reassessments through regular contact with clients to appraise changing needs and progress towards goals. Such contact should consist of home visits, client telephone conferences, client office visits and visits to other community locations.

Assist consumers in completing appropriate forms to ensure their basic financial needs are met. (Medicaid, SSI, SSD, food stamps, GA, etc.)

Responds to client-defined crisis as needed. Consults with supervisor regarding each situation before taking action.

Transports clients as needed in agency vehicles.



  • BA with 1 year experience working with individuals those are homeless or at-risk of homelessness
  • Experience in one of the following: crisis intervention, counseling, mental health, substance abuse, domestic violence, housing procurement, or job development
  • Must demonstrate good organizational, communication, negotiating and problem solving skills.
  • Must be a team player
  • Experience documenting services in computerized records.
  • Use of own vehicle
  • Possess a valid driver's license, insurable under Agency's automobile insurance policy
