Overview

CarePlus New Jersey is a northern New Jersey provider of recovery-focused mental health, substance abuse, and family services that address the unique needs of individuals with a holistic approach to recovery and overall wellness.

 

Operating out of 23 sites – including three outpatient centers located in Paramus, Fair Lawn, and Montclair, ten residential facilities, offices at three local hospitals, and seven community offices – our services are convenient to clients located throughout Northern New Jersey. We also provide educational programs on mental health-related topics to area schools and community groups.

Responsibilities

  • Engage consumers and their families in services
  • Facilitate and coordinate psycho-educational services (including groups) for consumers and their family members on a variety of topics relating to wellness (physical and mental), health promotion, prevention, skill-building and related health and wellness/recovery topics
  • Offer on-site as well as off-site outreach services (including transport of consumers)
  • Fulfill case management services, including, but not limited to coordinating or directly facilitating linkages to services, service coordination, treatment enrollment assistance, and so forth
  • Ensure consumers have community supports in place (and engage in service planning as appropriate) that fulfill a positive corroborative role
  • Communicate observations and information about the consumer in the consumer’s service record and to team members
  • Engage in supportive counseling, crisis intervention
  • Assess/evaluate consumers at intake/admissions, during ongoing recovery/service delivery and at discharge
  • Collaborate with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments and care plans
  • Formulates consumer-centered care plans in collaboration with team members, containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates effectiveness of goals/objectives
  • Works with administrative staff to ensure that all services are documented based on billing and regulatory standards; collects information on consumers' insurance and reflects all demographic data
  • Participates in quality assurance activities 

 This is a Per Diem position!

Qualifications

  • MSW or MA in related field OR

  • BA plus two years experience working within a healthcare or behavioral health setting

  • Possess a valid driver’s license, and must be insurable under Agency's automobile insurance policy

  • Excellent computer skills
